Extract cd prefixes in terminal tool and present separately

It doesn't happen so much with regular agent mode, but copilot/codex/etc. CLIs running inside chat will constantly do cd <dir> && prefixes. This makes reading the command a lot harder. We should pull it out and present it differently.
