Proof of the Philippine Citizenship
ie: Certificate of Naturalization, Voter's Affidavit/ID Card,
Philippine Passport/Visa
Birth Certificate or Marriage Contract if Married Female
Transcript of Records with Name of Degree, Date of Graduation and S.O
(B) Number Unless Exempt Therefrom Due to Accreditation of Course by
CHED or to Being a state School/College/University Graduate.
