4 Student Response and Assessment Template

This document describes a student response system (SRS) activity that will be used to formatively assess 10th grade math students' understanding of algebra standards after completing a unit. The teacher will introduce Socrative and give students an 18 question quiz to take using their devices. The purpose is formative assessment to help differentiate instruction. Sample multiple choice and open-ended questions are provided. Immediate feedback will be given and individual results will be shared to help students monitor progress. The teacher hopes to learn what concepts need more explanation so they can improve instruction.

Tablet (such as iPad)
Other wireless device (such as iPod
Touch)
Describe the instructional activities that will occur prior to the SRS
Activity and how you will introduce the SRS Activity: The SRS activity
will be used to review and assess student understanding of the content
after the unit of algebra is covered. I will introduce the SRS activity by
showing how Socrative works and giving students 18 quizes to assess
their learning.
Describe the purpose of the SRS activity (Check all that apply)
Assess Prior Knowledge
Anticipatory Set (Create Interest in a Topic)
To
Illuminate Common Misconceptions
Formative Assessment of Content
Knowledge (for purposes of differentiation and mastery for ALL students!)
Summative Assessment of Content Knowledge

Additional explanation of purpose (optional): type here
Type of session:

Teacher-Paced

Student-Paced

Identification of students:
Students will log-in so their individual scores will be available to you after the
session
SRS Activity will be anonymous
Briefly describe what will happen during the SRS Activity (For example:
What will students do? What will teacher do? What materials are needed? How
long will the SRS activity take?):
Students will log in to their account and do the quiz. Teacher will walk around and
provide guidance if needed. The SRS activity will take about one class session,
and students can continue to do it at home if they cannot finish in class.

Provide samples of questions/prompts to be given to students:
Which line given by its equation below contains the points (1, -1) and (3, 5)?
A -2y -6x = 0
B 2y = 6x - 8
C y = 3x + 4
D y = -3x + 4
Factor the algebraic expression 6x2 - 21xy + 8xz - 28yz.
Right/Wrong Answers: Will there be right/wrong answers to these questions? :
Yes
No
Mixed (Some will have correct answers, others will not.)
Immediate Corrective Feedback:
Will you pre-select correct answers to some or all of the questions and display the
correct response to the class after the SRS activity?
Yes
No
Why or why not? If students do not answer correctly, it is important that they
know the correct answer, so they can try it again immediately and find why they
get it wrong.
Use of Data: What data will be collected as a result of this activity. How will it be
used and by whom? (For example: Will information collected from this activity be
used to award a grade? Will the individual information collected be shared with
students and/or parents to help them monitor individual progress? Will you
discuss the aggregate, anonymous data with the whole class to help them learn?
Will you use data to differentiate instruction for students? If so, describe how. )
The data collected will be used by the teacher to determine which areas need
more review before the unit test. The individual information collected will be
shared with students and parents to help them monitor individual progress. I will
discuss the aggregate, anonymous data with the whole class to help them
understand. I will aslo use the data to differentiate instruction for students by
providing several learning options, or different paths to learning, which help
students take in information and make sense of concepts and skills as well as
providing appropriate levels of challenge for all students, including those who lag
behind, those who are advanced, and those right in the middle.
Describe what will occur after the SRS activity: After SRS activity, feedback,
discussion and review will be provided to help students understand the content.
Also, similar questions will be included in the homework for students to do more
practice at home.
Describe your personal learning goal for this activity. (For example: What
are you trying that you have not tried before? What do you hope to learn from
this activity? How do you hope it will help students learn? You must design
something that will help you learn something new! Honor System!) I hope that I
will learn the areas that students need help which would be the weak parts of my
instruction. Then I will try different methods to imporve my instruction or spend
more time on the concepts that students are struggling.
Other comments about your SRS Activity (optional): I think Socrative is a
great tool for students to review what they have learned and for teachers to know
the areas that students need more time to review or practice before the test or

continuing the new session.
