Instructions for initial registration to the Professional Regulation Commission (PRC) for new professionals who have just

passed the nursing board exam (Nurse LicensureExamination):
Initial registrants should come personally to the PRC Central Office or to the nearest Regional Office to file his/her application
for registration and to affix his/her signature in the registry book. (INITIAL REGISTRATIONSCHEDULE FOR NEW R.N.’s
TO BE ANNOUNCED SOON BY THE PRC)
PRC Initial Registration Schedule for new R.N.’s: Cordillera Administrative Region, Region I and Region III
REQUIREMENTS DURING INITIAL REGISTRATION:
1. Duly-accomplished Oath Form or “Panunumpa ng Propesyonal” form. Go here to download the Oath Form or
“Panunumpa ng Propesyonal”.
2. Current Community Tax Certificate (cedula).
3. Two (2) pcs. passport size pictures with name tag (for Oath Form and Certificate of Registration).
4. One (1) pc. 1×1 picture in plain background with complete name tag (for Registry of Professionals).
5. Two (2) sets of metered documentary stamps (for Oath Form and Certificate of Registration).
6. One (1) short brown envelope with name and profession.
7. Initial Registration Fee of PhP 600 (six hundred pesos).
8. Annual Registration Fee of PhP 450 (four hundred fifty pesos) for 2010 – 2013.

INITIAL REGISTRATION PROCEDURE:
Step 1. Accomplish the Oath Form or “Panunumpa ng Propesyonal”.
Step 2. Pay for two sets of metered documentary stamps (one stamped on this form and one for theRegistration Certificate) at the
Customer Service Center.
Step 3. Present duly accomplished oath form (where your profession belongs, for verification of records and assessment at
Window 14, Window 19, Window 28 or at PRC Service Center, Ever-Recto.
Step 4. Pay prescribed fees at the Cashier.
Step 5. Sign in the Registry Book and receive your claim slip at: Window 17, Window 20, Window 27.
Step 6. Claim your professional ID as scheduled. Refer to your claim slip for further instructions.
Certificates and PRC ID shall be claimed by the licensee personally at the Central Office or the Regional Office where the
application has been filed. If there is an unavoidable need for a representative, the representative must present
his/her identification paper with picture and aSpecial Power of Attorney.
