Weekly Action Plan (WAP) Objectives of the day (October 2-3, 2012) Obtain necessary information needed about

the different patients admitted. Plan of Activities Be able to attend the endorsement Be able to read the patients chart to have an overview on the condition of each patient. Be able to analyze the information and identify the initial problem or condition of the patient. Be prepared in any situation that might be encountering and be alert. Be aware of whats happening on the patient. Plans with members what to do, what to expect and what to avoid for quality nursing care. Key Areas of Responsibility Records Management, Personal and Professional Development, Collaboration and Teamwork, Legal Responsibility, EthicoMoral Responsibility. Remarks

Determine the goals or any future plans for the whole day of duty.

Communication, Collaboration and Teamwork, Quality Improvement, Research, Safe and Quality Nursing Care, Management of Resources and Environment, Personal and Professional Development.

Should need more cooperation and organization among all the members to have productive outcomes.

Provide nursing care to the patient.

Provide safety and comfort to the patient while performing any nursing intervention. Maintain the privacy of the patient. Establish rapport with the patient and significant others. Conduct necessary nursing interventions on time like taking vital signs, giving medications. Be able to provide resources for the continuity of care or for the safety of the patient if certain resources are not available or if facilities are damaged. Do necessary interventions for unexpected situations like fever.

Be liable for any action done and demonstrate good conduct at all times.

Continue conducting health teachings about Breastfeeding to each patient for their own self awareness.

Document relevant data and care given to the patient.

Establish rapport with the patient. Consider the clients preparedness and behavior towards the health teaching. Completes documentation of client care. Endorse to an appropriate person/s any variance or changes regarding the patients condition.
