xxx : Email ATF License x-xx-xxx-20-xx-xxxxx These worksheets have been developed to meet the requirements set forth by the Bureau of Alcohol, Tobacco and Firearms (ATF) for recordkeeping for explosives transactions, as defined in Title 27, Code of Federal Regulations, Part 55, Subpart G -- Records and Reports. The worksheets defined in this file are oriented toward manufacturers of explosives, however dealers, importers, and users should be able to modify these easily for their own purposes. To maintain your records on a computer, you will need to request a varience (in writing) from your local ATF office. Each type of worksheet, and the fields it contains, is explained below. All of the information used in these worksheets is required per the ATF Orange Book. If you have questions about these records, contact your local ATF field office. Record of Manufacture of Explosives The purpose of this worksheet is to maintain a record of each and every item manufactured by the licensee. This requirement is defined in section 55.123(b) in the ATF Orange Book, Page 32. Seq Sequence number for this entry. Mfgr ID The ATF requires each manufacturer to assign a manufacturers ID to each item manufactured. This can be an item code, serial number, or some other mark to identify the item. Date The date the item was manufactured. Quantity The number of units manufactured. Units The quantity units, including each, dozen, lbs, ounces, sticks, cases, etc. Description A description of the item manufactured, for example rocket, ball shell, gerb, etc. Size For fireworks items, this is the size of the item, such as 6" for a 6" ball shell. Location of Manufacture The general location of where the item was manufactured. Record of Receipt or Purchase of Explosives The purpose of this worksheet is to maintain a record of each item you acquire, either through purchase or transfer. This requirement is defined in section 55.125(b) in the ATF Orange Book, Page 34. Seq Date Mfgr Name Mfgr ID Sequence number for this entry. The date the item was purchased or acquired. The name of the manufacturer of this item. The item number, serial number, or other mark used by the manufacturer to identify this item.

Quantity The quantity of items acquired. Units The quantity units, including each, dozen, lbs, ounces, sticks, cases, etc. Description A description of the item acquired, for example rocket, ball shell, gerb, etc. Size For fireworks items, this is the size of the item, such as 6" for a 6" ball shell. Name of Supplier The name of the supplier where the item was acquired. Supplier Address The address of the supplier. ATF Permit Number The ATF permit number of the supplier.

Record of Transfer or Sale of Explosives The purpose of this worksheet is to maintain a record of each item you sell or transfer to another individual or company. This requirement is defined in section 55.123(c) in the ATF Orange Book, Page 32. Seq Sequence number for this entry. Date The date the item was sold, transferred, confiscated, or stolen. Mfgr ID The item number, serial number, or other mark used by the manufacturer to identify this item. Quantity The quantity of items sold, transferred, stolen, or confiscated. Units The quantity units, including each, dozen, lbs, ounces, sticks, cases, etc. Description A description of the item transferred, for example rocket, ball shell, gerb, etc. Size For fireworks items, this is the size of the item, such as 6" for a 6" ball shell. Recipient Name The name of the recipient of this item. ATF Permit Number The ATF permit number of the recipient. Record of Use of Explosives The purpose of this worksheet is to maintain a record of each explosive item that is used or disposed of, including tests of manufactured items. This requirement is defined in section 55.123(d) in the ATF Orange Book, Page 32. Sequence number for this entry. The date the item was used. The quantity of items used or disposed of. The quantity units, including each, dozen, lbs, ounces, sticks, cases, etc. Description A description of the item used or disposed of, for example rocket, ball shell, gerb, etc. Size For fireworks items, this is the size of the item, such as 6" for a 6" ball shell. Record of Magazine Inventory The purpose of this worksheet is to maintain a record of each item that is transferred into, and out of, the magazine. Separate records must be maintained for additions, subtractions, and a summary of the current magazine contents. Each magazine requires a separate worksheet. Magazines that are shared by multiple license holders require that each licensee maintain their own records for their own items. The requirement for magazine inventoriy records is defined in section 55.127 in the ATF Orange Book, Page 35. Seq Sequence number for this entry. Date The date the item was entered into the magazine inventory. Quantity The quantity of items reflected in this magazine transaction. Units The quantity units, including each, dozen, lbs, ounces, sticks, cases, etc. Mfgr Name The name of the manufacturer of the item. Description A description of the item, for example rocket, ball shell, gerb, etc. Size For fireworks items, this is the size of the item, such as 6" for a 6" ball shell. Weight The estimated weight of the item(s) in pounds. Used for calculating total magazine weight. Source For items added to the magazine, this is the source of those items (where they came from). Destination For items that are transferred out of the magazine, this is the destination for those items. Comments This is for optional comments regarding items in the magazine. Record of Magazine Inspection The purpose of this worksheet is to maintain a record of each time a magazine is inspected. These inspections must be performed at least every seven days. While record keeping for this activity is not required, the ATF recommends that these inspections be recorded for compliance reasons. The requirement for magazine inspections is defined in section 55.204 in the ATF Orange Book, Page 40. You may want to record onsite ATF visits and/or inspections in this section as well. Seq Sequence number for this entry. Date The date the magazine was inspected. Inspection Type The type of inspection, including spot check, full inventory, or ATF inspection. Comments This is for optional comments regarding the inspection of the magazine.
