AND WAIVER FORM must be accomplished.

PARENTAL CONSENT AND WAIVER FORM

I, ________________________________, grant permission for my son/daughter,


_______________________________ to participate in the Youth Leadership Training
Program which requires travel to a location outside the Tuguegarao City Hall. This
activity will take place under the guidance and direction of the Office of the City
Mayor of the Local Government Unit of Tuguegarao City.

A brief description of the training follows:


Location of Event: Caronan Farm Resort, Cabbo, Peablanca,
Cagayan
Date and Estimated time of arrival/departure:
Arrival November 30, 2017, 8:00 AM
Departure December 1, 2017, 5:00 PM
Mode of Transportation: LGU Bus

I, as parent and/or legal guardian, remain legally responsible for any personal actions
taken by the above named participant. I agree on behalf of myself, my child named
herein, or our heirs, successors and assigns, to hold unaccountable and exclude the
Local Government Unit of Tuguegarao City, its officials, employees, facilitators or
representatives associated with the event, from any and all actions, claims, demands,
damages, costs, expenses and all consequential damage arising from or in connection
with my child attending the event or in connection with any illness or injury or cost
of medical treatment in connection therewith.

In signing the Parental Consent and Waiver, I am not relying on any oral or written
representation or statements made by the LGU and , its officials, employees,
facilitators or representatives associated with the event, to induce me to permit my
son/daughter to take the training program, other than those set out in this Consent
and Waiver.

I have read and I understand the terms of this consent and waiver, and I recognize
that it is binding upon me, my heirs, executors and administrators.
