PCNSL

The biopsy shows angiocentricity and angioinvasion of tumor cells within and around cerebral blood vessels at both low and high power, with areas of vascular necrosis. At low power, there is extensive infiltration of the field and concentric perivascular reticulin deposits. At high power, there is perivascular growth of large atypical cells diffusely with a "starry-sky" appearance and infiltration of brain parenchyma as clusters and individual cells, along with atypical chromatin. Immunohistochemistry staining is positive for CD20 and BCL2, supporting a diagnosis of diffuse large B-cell lymphoma (DLBCL).

INTERPRET BIOPSY

At both LPO and HPO the diagnosis is clearly apparent as it shows angiocentricity and angioinvasion
(focal with vascular necrosis- yellow circle) which forms cuff of tumor cells within and around cerebral
blood vessel.

At LPO, there is extensively infiltration of the field and there is perivascular reticulin deposits (ung
parang shade ng red na pumapalibot sa vessel tama ba? Green circle) in a concentric fashion. (?).
AT HPO, it clearly shows perivascular growth of large atypical cells, the distribution is diffuse and “starry-
sky” appearance without perineural satellitosis. The tumor infiltrates the brain parenchyma mostly as
clusters (blue circle) and as individual cells. It also revelas higly atypical coarsely granular chromatin, and
in some nucleus, with prominent nuclei.

IMMUNOHISTOCHEMISTRY

- IHC stain with antibodies against CD20+, BCL2+ typically supports the diagnosis for B cell
lymphoma (DLBCL)
