AFFIDAVIT OF ACCEPTANCE

I, ALDEN M. AZIZ, of legal age, Filipino, married and a resident of


Linusungan, San Marcelino, Zambales, being first duly sworn in
accordance with law, depose and state that:

1. I am the transferee of a parcel of land located at San Isidro, San


Marcelino, Zambales containing an area of NINE THOUSAND
THREE HUNDRED FIFTY (9,350) square meters declared for
taxation under Tax Declaration No. 016-0568;

2. That said parcel of land has no pending case before the


Department of Agrarian Reform Adjudication Board and/or any
other court;

3. That the transferor thereof is no longer interested to till the land


and unable to perform the duties and obligations for the above-
mentioned land;

4. I hereby accept the said transfer unequivocally and without


condition and I am willing to perform all the duties and
responsibilities and agricultural tenets as provided by pertinent
tenancy laws on the above mentioned parcel of land; and

5. I am executing this affidavit to attest to the truth and veracity of


the foregoing facts and for whatever legal purpose it may serve.
