DESCRIPTION:

On 25th October 2018 at 14:30 got an information from Mr.Amanullah Ghazi (QC piping
inspector) of technimont Arabia about incident of RTR 2inch pipe burst on 14.8 bar pressure
near joint # 9 at north side of sru sewer line near structure F area during hydrotest activity
which was continues from 12:30 by hydrotest forman named Ramazan Kural without obtaining
work permit. No personnel injury reported.
IMMEDIATE ADOPTED MEASURES:

Immediately stop the activity.

Given warning letter to activity Forman regarding to start the work without obtaining work
permit.
