I. SCOPE/ PURPOSE
The purpose of this document is to help the registrar personnel in assessing clients
requesting their respective Transcript of Records for employment purposes. This serve as their
guide or instructions in doing their specific jobs properly without committing mistakes.
Through this document also, it will help the client/ customer to be inform in what are the things
to do as a person requesting their TOR. This process is limited only to those clients requesting
their TOR for employment purposes only.
II. REGISTRAR POLICIES:
1. Client should have a complete record (F- 137, F-138, NSO/ BIRTH CERTIFICATE/
GOOD MORAL from Secondary) on his/her file. If client doesn’t have complete record,
then Student Assistant at Registrar Office will make a request form for such record (F- 137,
F-138, NSO/ BIRTH CERTIFICATE/ GOOD MORAL from Secondary) and give it to
client.
2. It’s the client obligation to forward the request form to his/ her respective secondary school
so that the principal or the guidance counselor will issue the lack records.
3. Client should have a complete grade as well. If the client doesn’t have complete grades,
then he/ she will settle it with his college instructor.
4. Its clients’ obligation to sign his/ her clearance to respective persons like the Adviser,
College DEAN, University Cashier and University President as well.
5. The clients must fill up completely the request form.
III. PROCEDURE
