KNOW ALL MEN BY THESE PRESENTS:

I, ______________ , single, of legal age, Filipinos and all residing at


_______________, do hereby name, constitute and appoint and by this presents has
NAMED, CONSTITUTED and APPOINTED, ________, likewise of legal age, Filipino,

1. To negotiate and to enter into a Lease Agreement with Dito Telecommunity


Corporation (“Dito”), over portions of a parcel of land (the “Lot”) located at

Transfer Certificate of Title (TCT) No. ___________ declared under Tax
Declaration No. ____________ on such terms and conditions as may be
deemed reasonable by my said Attorney-in-fact;

2. To sign the Lease Agreement and to deliver any papers/documents relative to


the lease of the premises to Dito; and

3. To accept the check to be issued in the name of my Attorney-in-fact by Dito


representing rentals thereof and other charges, to encash the same and issue
acknowledgement receipt therefor.

HEREBY GIVING AND GRANTING unto my Attorney-in-Fact full power and


authority whatsoever requisite or necessary or proper to be done in or about the
premises, as fully to all intents and purposes as I might or could lawfully do if
personally present and hereby RATIFYING AND CONFIRMING all that my Attorney-
in-Fact shall do or cause to be done under and by virtue of these presents.
