I, _________________________________ hereby give my
permission to Brunswick Community College Performing Arts Club to be
photographed of filmed for use by the BCC Performing Arts Club to be
photographed or filmed for use by the BCC Performing Arts Club in
programs, promotions, public relations, or advertising for the
Performing Arts Club productions or performances, I also understand
that audio recordings of my voice can be used for said purposes.

I give further consent to use the above materials at Brunswick


Community College for such purposes as print, slideshows, video, or
other future promotions of Performing Arts Club activities.

I give consent to any outside photographers or videographers to


record, copy, print, or distribute my image(s) in association with the
Brunswick Community College Performing Arts Club.
