Legend (Rating Criteria):
5 – Expert (student performs all tasks proficiently and independently).
4 – Competent (student performs efficiently in an effective and efficient manner).
3 – Progress Acceptable (performance is usually effective and but not always).
2 – Needs Improvement (progress in performance is too slow to judge satisfactorily; task
performance is not most of the time).
1 – Progress Unacceptable (no progress in performance has been demonstrated, and or
performance is consistently ineffective and inefficient).
Fundal massage, also called uterine massage, is a technique used to reduce bleeding and
cramping of the uterus after childbirth/delivery or after an abortion. As the uterus returns
to its nonpregnant size, its muscles contract strongly, which can cause pain.
Purpose:
1. To contract the fundus and prevent/stop bleeding.
2. To enhance uterine involution.
3. To expel blood clots and placental fragments.

1. Ensure clean and warm hands to promote safety and comfort.
2. Explain the procedure to the patient.
3. Have the client void before the procedure.
4. Place the client in supine position with slight knee flexion (dorsal
recumbent).
5. Stand on the side of the bed at the level of the mother’s
umbilicus.
6. Feel the uterine fundus lightly and gently with fingertips.
7. Inform the client that he/she will massage her uterus.
8. Place one hand over the symphysis pubis and the other hand on
the fundal area.
 Notes and feels the fundal location. Feels the fundus and
notes its consistency: firm, boggy, soft or non-palpable.
9. Massage the soft fundus until it contracts using smooth, steady,
downward, circular motion.
 Discontinues fundal massage if the uterus is contracted.
10. Empties the uterus when it is adequately contracted.
11. Teaches the client how to palpate and massage her uterus.
 Explain to the client that uterine contraction causes some
discomfort and that it is a normal sign of uterine involution.
12. Informs the client that breastfeeding causes the uterus to contract
and that it has the same effect as fundal massage.
13. Promotes client comfort after replaces or changes perineal pad
as needed; covers the client.
14. Does proper recording and reporting.
