Nurse : Good morning, Mrs. Chad. My name's Shona. I'll be admitted you to the ward today.

Patient : Good morning, Shona.

Nurse : How are you today?

Patient : Not to bad, thank you.

Nurse : I'd like to ask you some questions, if it's all right with you?

Patient : Yes, of course. That's fine.

Nurse : Can you tell me your name, please?

Patient : Yes, it's Doreen Mary Chad.

Nurse : Can you tell me your date of birth?

Patient : My date of birth is the fifth of June nineteen twenty-three.

Nurse : Can you tell me why you are here?

Patient : Well, I've got high blood pressure and I'm here for some tests.

Nurse : Have you had serious illnesses in the past?

Patient : Yes, I had a mild heart attack last year.

Nurse : Have you ever had any operations?

Patient : No, I'm very lucky. I never have.

Nurse : Now, are you taking any medications at the moment?

Patient : Yes, my doctor put me on some blood pressure tablets after my heart attack.

Nurse : Do you have any allergies to any medications?

Patient : Not that I know of.

Nurse : Can you tell me the name of your next kin?

Patient : It's my son, Jeremy. Jeremy Chad.

Nurse : Thanks. That's all for me. I'll leave you here for a minute while I'm getting the admitting doctor to
come and see you.

Patient : Yes, thanks.

Nurse : Hello. It's Mr. Connolly isn't it?

Patient : Yes, that's right.

Nurse : My name's Stephen. I'm looking after you today.

Patient : Oh, Stephen. Right.

Nurse : Here's the nurse call. Just press the button if you need some help.

Patient : Oh, all right.

Nurse : Do you need help to walk to the bathroom?

Patient : Yes, I'm a bit unsteady on my feet.

Nurse : That's okay. Just call me and I'll help you.

Patient : Thanks, Stephen.

Nurse : Is there anything else you need?

Patient : Err, no I'm fine. Thanks.

Nurse : Mr. Connoly , have you got an ID bracelet on?

Patient : Yes, here it is.

Nurse : Can I look at your ID bracelet, please?

Patient : Certainly.

Nurse : Can you tell me your full name please?

Patient : John Everson Connolly.

Nurse : Right. That's correct on the bracelet. What's your date of birth, please?

Patient : The thirthieth of November ninteen thirty-four.

Nurse : correct. Your hospital number is ... (nurse read the hospital number and check it with the patient
information document)

Patient : ...

Nurse : Do you have any allergies?

Patient : ...

Nurse: Okay, ...
