Dear Mayor:

In the event that the preparation of the list of voters in the forthcoming
May 13, 2013, Synchronized National and Local elections is ongoing and
considering the voluminous works in our office during this season.

In view of the above, may I respectfully request two (2) casuals effective
November 1, 2012 chargeable against the 2 nd Augmentation Budget of the Local
Government Unit (LGU) this quarter, if not too much, an office supplies worth of Ph
1,000.00 to be used In our office.
