Rheumatoid Arthritis Diagnosis Insights

A 4 year old female presented with a several month history of limping and swelling of her right knee. On examination, she had swelling of the right knee with a flexion contracture of 10 degrees and ability to flex to 120 degrees. Laboratory tests found an elevated ESR but negative rheumatoid factor and positive ANA. The most probable diagnosis is juvenile rheumatoid arthritis.
