perjury, that we are ready willing and able to buy the following goods under the terms and
conditions stated herein:

PACKAGING: Box of 10 per Carton or as declared by the Seller
REQUIREMENTS: All the gloves offered must have Category III as personal protective
equipment (PPE) and must therefore bear the CE mark for a dual product certifying its use as a
medical device and as personal protective equipment.
SPECIFICATIONS: Nitrile Powder Free examination gloves Medical EN455, ISO 9001 OR
EN 13485 certifications from the factory
CERTIFICATIONS: PLEASE MENTION

This ICPO is the only current active ICPO and has been exclusively offered to Seller for
the release of the Draft Contract. This ICPO voids any and all other ICPOs currently in
circulation for this quantity and product.
