ADDENDUM TO THE CONTRACT OF SERVICES
This Addendum to the Service Contract (“Addendum”) entered into this ___ day of ___________,
20___ at Valenzuela City by and between:
ARDY MANUFACTURING CORPORATION, a corporation
duly organized and existing under Philippines Laws, with principal
office address at 6A L. San Diego Street, Canumay West, Valenzuela
City, represented herein by its President, MR. ARTHUR C. DY, and
hereinafter referred to as the “CLIENT”,
-and-
ECTAG MANPOWER SERVICES, a single proprietorship
established under Philippines Laws, with office address at Ground
Floor, Dulatas Bldg., Maysan Road, Valenzuela City, represented
herein by its Manager, MR. RODOLF ALEXANDER G. RAMOS,
hereinafter referred to as the “CONTRACTOR”.
WITNESSETH:
WHEREAS, the CONTRACTOR agreed to perform and provide to the CLIENT desired services
that the latter might need during the term of the contract.
WHEREAS, the CLIENT, hereby leases unto the CONTRACTOR, and the latter hereby accepts
to lease from the former, the following equipment under the following terms and conditions:
LIST OF MACHINERIES AND EQUIPMENTS

1. 43 Units Mechanical Power Press
Can Assembly Line Machines
2. 7 Sets (Welding Machine, Seaming Machine, Flanging Machine, Strapping
Machine)
3. 3 Units Spot Welding Machine
4. 7 Units Rubber Lining Machines
NOW, THEREFORE, for and in consideration of the foregoing premises, the Parties have
expressly agreed upon the following terms and conditions of this Addendum:
1. The monthly rental shall be ONE HUNDRED THOUSAND (Php100,000.00), Philippine
currency to be deducted on the billing.
2. The terms of the lease is for ______ (____) year commencing on ____________________ and expiring
on __________________________.
3. The CLIENT hereby expressly agrees and warrants that the leased equipment and premium
shall be used exclusively for use in selling, to be contracted from CLIENT. And the said
CONTRACTOR is hereby strictly prohibited from using the said equipment for any other
purpose without the prior written consent of the CLIENT.
