Secretary’s Certificate

This is to certify that as per minutes of the BAC meeting held on __________, the
Committee resolved to recommend to the HoPE, the approval of the extension of the following
contracts for the C.Y. 2021, to wit:
1. xxx xxx
2. xxx xxx
3. xxx xxx;

On the same date at above-mentioned, the HoPE approved the recommendation of the
BAC to extend the foregoing contracts;
Moreover, the Committee resolved to fast track the procurement process of the
aforementioned contracts;
Further, that a written Board Resolution could not be issued or released for the reason
that the PRO Caraga office is currently on a lockdown due to Covid-19, which rendered the
impossibility on the part of the BAC members to physically report for work and sign the afore-
stated resolution;
Finally, this certification is issued in order to temporarily comply with the request of the
Area IV ???, for the submission of the contracts for the C.Y. 2021 and that a Board Resolution
will be submitted upon resumption of work, and ultimately, so that a formal contract may be
finalized, approved and completed immediately thereafter.
