Briquette Processing Plant

Unsafe activity: On 26.06.21 it has been observed that Forklift of M/s. Vaaman Engineers (I) Ltd.

No OD23 H 1418 which was operating by Mr. Rupesh chhatar. At around 4:30 PM he left the

forklift in Engine ignition mode and charging the Briquettes Jumbo bag at auto bagging conveyor.

Abruptly the forklift operator Mr. Rupesh left the forklift and doing some briquette cleaning

activity by non-standard TMT rod. Ref. below Pic. No. 1

Consequence: There was poor supervision by site and shift in charge in where it leads to LTI or

Major injury as the person movement around line of fire and not followed 2 Mtr Rule.

Unsafe activity: On 26.06.21 it has been observed that Electrical technician Mr. Subham and

Mr. Tapas, carried out maintenance activity with out isolating the energy as per the LOTOV and

both are not wearing electrical insulation class 00 type hand gloves at site for the work. Ref. below

Consequence: There was poor supervision by site and Electrical in charge in where it leads to

LTI or Fatality as the person not wear insulated hand gloves and not followed LOTOV .

Unsafe activity: On 28.06.21 it has been observed that Mr. Pallab Ku. Jenamani and Mr. Iswar ,

carried out cleaning activity near tansit mixer conveyor belt where Mr. Iswar found standing on

the conveyor belt and doing the cleanng job with out isolation. Ref. below Pic. No. 5

Consequence: There was poor supervision by site in charge in where it leads to LTI or major

injury as the person not followed Isolation and may slip/trip due failure of balance.

We have been strictly insisting for safe working by highlighting concerns at all forums but the

lapses are still found with repetitive violations.

Note: If any repetition observed in future, Disciplinary action will be taken on Personnel who has
been involved in the unsafe acts & unsafe conditions.
We once again insist for maintaining highest safety standards without any compromise.
Let’s improve safety standards prevail at workplace to achieve Zero harm, Zero tolerance, Zero
discharge and Zero waste.
