Member completes inpatient GL request form for
admission/inpatient treatment in Panel Hospital
Member emails or fax completed request form to
IHP 3-5 days prior to visit date
Further documentation/ information may be requested
to facilitate claims adjudication if required.
IHP to email Member and Panel Hospital on GL issuance
Member visits Panel Hospital and present the following:
1. IHP card/e-card Medical Services Rendered
2. NRIC/ Passport
3. Guarantee Letter
Access To Inpatient Care
Emergency Admission
IHP Panel Hospitals in Malaysia
(via Inpatient Guarantee Letter)
Member visits Panel Hospital
and presents IHP Medical Card/ E-Card
Panel Hospital contacts IHP with the relevant supporting
documents for GL assessment and issuance
Further documentation/ information may be requested
to facilitate claims adjudication if required.
IHP issues Inpatient GL to Panel Hospital
Medical Services Rendered
Reimbursement Claim Submission
Reimbursement Claim submission
Member submits claim form, original claim invoices
and relevant supporting documents to Zurich:
1. Hospital Final Invoice
2. Doctor’s Memo/ Referral Letter
3. Medical Reports
IHP will process/ adjudicate claims in accordance to policy
guidelines
Further documentation/ information may be requested to facilitate
claims adjudication if required.
IHP to advise member on claims settlement
