Six Only) of myself.
Dear Sir,
I regret to inform you that on 15/10/2013 afternoon suddenly I felt very sick & consulted
with Dr. Razib Bishwash at Pacific Health Care Center, Feni. As per his advise I got
admitted into Feni Diabetes Hospital on 18-10-2013. They discharge me on that day
advised me to get admission in Chittagong Medical College Hospital at the Medice
Ward. They analyze different test result of mine & found that I was suffering from
Dengue Haemorrhagic Fever. Than they prescribed me some medicine & advised me to
stay there for their observation. After the improvement of my physical condition they
released me on 23/10/2013.
After that I consulted with Dr. [Link] at Al-Kemi Hospital, Feni to check my
physical condition. He also advised me to take rest & prescribe some medicine which I
continued for the prescribed period.
Now my physical condition is quite ok.
At that emergency situation, I could not obtain permission from your end. Considering
the circumstances kindly accord your post facto permission & reimbursement of the
medical bill for Tk.= 10,456/- (Ten Thousand Four Hundred Fifty Six Only) for which I
shall be highly obliged.
