AFFIDAVIT

I, RODERICK A. ARIGO, of legal age, Filipino citizen and with address at


Barangay Amaya, Tanza, Cavite, after having been sworn in accordance with law, hereby depose
and state that:

1. I currently applying for the issuance of Certificate of Non-Coverage (CNC) before the
Department of Environment and Natural Resources;

2. The aforesaid issuance of CNC is for the purpose of establishing that I am not and
will never be engaging in any trading of hazardous material;

3. In compliance of the said CNC, I hereby undertake and make the corresponding
warranty that I will not engage in the buying, selling and trading of hazardous
materials and any activities not in consonance with the CNC;

4. I am executing this affidavit to attest to the truth of the foregoing statements and for
any legal purpose this affidavit may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 11th day of February, 2014,
in the Municipality of Tanza, Province of Cavite, Philippines.
