1. PURPOSE
The purpose of implementing a Permit to Work (PTW) is to monitor all potential hazards from high-risk
work activities to ensure that proper controls have been put in place so that it takes place safely.
2. SCOPE
This work permit procedure must be applied for high-risk work activities carried out by
employees/contractors of PT. Indorama Ventures Sustainable Solution Indonesia.
3. DEFINITION
3.1. Hot work is work carried out in hot areas with temperatures above room temperature or work that
generates heat or sparks.
3.2. Work Permit Form is a document or written permit that is used to control certain types of work that
have the potential to endanger workers.
3.3. Confined Space is a work permit that is given to any work in a closed or partially closed area, which
is not designed for a work area and is prohibited as an entrance and exit and has potential hazards
including, but not limited to contaminated air/atmosphere and levels low oxygen.
3.4. Work at Height is an activity or work activity carried out by workers in a workplace where there is a
difference in height and has the potential to fall which causes the worker or other people to be
injured or killed and cause losses.
3.5. Personal Protective Equipment (PPE) is equipment that is used when working in accordance with
the hazards and risks that exist to protect and or reduce exposure to hazards/risks in the workforce in
the workplace
3.6. Job Safety Analysis (JSA) is a process to identify sources of hazard at each stage of work, then
examine in depth to eliminate/control potential hazards as outlined in writing as work standards that
must be implemented consistently.
4. RESPONSIBILITY
4.1. HSE Departement responsible for :
a. Ensuring that work permit information can be communicated correctly and effectively.
b. Help analyze in making Job Safety Analyzes (JSA) for jobs.
c. Perform safety checks before, during and after work is carried out.
d. Carry out air testing for specific jobs and check general conditions in the workplace
4.2. The Job Executor is responsible for:
a. Identify the hazards of the work to be carried out.

b. Perform safe work procedures that must be followed


c. Use the specified Personal Protective Equipment.
d. Using safe equipment and in accordance with the type of work
4.3. The Employer/Supervisor is responsible for:
a. Know the type of work and the hazards of the work performed.
b. Knowing the number of workers doing the work
c. Know the equipment used.
d. Apply for a Work Permit and fill in the JSA addressed to the HSE Team in their respective
regions.
e. Carry out and comply with all the requirements set out in the work permit and its conditions
f. Provide workers with safe, appropriate and sufficient equipment, tools and PPE including
safe workplaces/facilities.
g. Explain to workers about the job and how to carry out work safely.
h. Notify workers immediately of any changes that could result in a hazard.
5. PROCEDURE
5.1. The types of work that require a work permit are as follows:
a. Work entering confined spaces such as cleaning tanks, cleaning sewers/gutches, excavation
work with a depth of 1.8 meters, working in rooms with limited ventilation etc.
b. Hot work such as welding, metal cutting, starting vehicles with internal combustion engines
in gas hazardous areas, sand blasting, using pneumatic tools etc.
c. Electrical work with a voltage of 1000 volts or more
d. Work at height above 1.8 meters such as roof repair, cable connection etc.
e. Lifting work using a crane, hoist or other mechanical lifting equipment.
5.2. Preparation stage
a. The employer/executor of high-risk work must apply for a work permit in writing and
include a Job Safety Analysis (JSA) for the work to be carried out to the HSE team using the
appropriate Work Permit form.
b. The HSE team will review the proposed work permit
c. based on conditions at the work site such as working environment conditions, work
equipment and other supporting equipment in accordance with work safety requirements
contained in the work permit form.

d. The party applying for a work permit must revise the JSA if discrepancies/lack of control are
found.
e. If the JSA evaluation and all requirements have been met, then the HSE team approves the
required work permit and provides an HSE briefing.
f. The HSE team will measure the air condition in the workplace (gas testing) using appropriate
equipment when necessary, for example for confined space and hot work.
5.3. Implementation Stage
a. Implementing workers must comply with all HSE regulations that have been determined
during work.
b. A copy of the work permit must be posted near the workplace and easily visible to other
workers.
c. For hot work, the job supervisor will supervise for the first 30 minutes, supervise regularly
during work and supervise 30 minutes after work is complete.
d. For work in confined spaces, workers must always be supervised while the work is in
progress.
e. Work may only be carried out at the workplace with a predetermined time. If discrepancies
are found during the work being carried out, the HSE team has the right to stop work until
the discrepancies are corrected.
f. The work permit is only valid for 1 time work in accordance with the provisions in the work
permit and a maximum of 1 day (8 working hours) in one work shift.
g. If the work is not completed within the specified time, the executor/applicant must stop the
work and re-apply for a work permit to the HSE team.
5.4. Completion Stage
If the work has been completed, the executor of the worker signs a work permit and submits it to
the HSE team to verify the safety and security of the work being carried out.
