I undersigned xxxx agree with SThree GmbH Switzerldfvfdvfdvand to represent me through

the WORKSENSE Program at Johnson & Johnson for the position of JJJP00014395 CSV
Engineer. I confirm that I have been sufficiently informed about the role (tasks, location,
duration, start/end date etc.) and the information I have provided is an accurate account of my
experience and skills.

I acknowledge that this document will be used to settle any dispute regarding my representation
for the above role according to the rules of the WORKSENSE Program, however this document
is not binding for me to accept the assignment if Johnson & Johnson wants to engage me.

I also understand that my candidacy may not be considered if I sign this document with more
than one supplier for the above role.
