Description: This monitoring tool is designed for used by supervisors ( EPS, PSDS & DIV.
COORDINATORS) who will visit the elementary and secondary schools, if they establish an
awareness ,perception and implementation of CSE in the integration to K to 12 Basic Education
Curriculum in the identified subject areas and key stages as mandated in DepEd order No. 31 s 2018.
