Instructions:
● Proponent: Tick the HSE requirements that are applicable to the scope of the Project or elements to be considered in HSE Plan.
Provide the accomplished form to the Contractor, and FOM-HSE (for Local Contractors) or HSE-BPMS (for NCR-based and
enterprise-wide projects). Use this to check whether the contractor’s Health and Safety Management Plan has the required
information based on the type of work that is expected to be carried out
● Contractor: Submit the HSE requirements as part of the bidding document to the Proponent. Costs that are required to deliver the
requirements should be included in the bid.
● Mode: Select the mode regarding the governance of HSE Management System in the following:
○ Mode 1 - Work activities where EDC can and is held accountable to fully influence and control HSE performance. (Typically
applies to services in any EDC location fleetwide, including brownfield project sites.)
○ Mode 2 - Work activities where EDC can and is held accountable to partially influence and control HSE performance. (Typically
applies to services in owner facilities, green field project sites and joint venture partner locations.)Mode 3 - Work activities where
