RA - Emcy Anchoring in Deep Waters.

This risk assessment form evaluates the risks associated with emergency anchoring in deep waters from the Lotus Shipping vessel. It identifies several high risks including inadequate planning, defective equipment, improper operations, adverse weather, and dangerous sea conditions. To mitigate these risks, the form recommends developing an anchoring plan, calculating the proper amount of chain based on conditions, using personal protective equipment, conducting a safety meeting, and walking out the anchor chain to avoid excessive loads. Following these control measures lowers the risk ratings from A/B to mostly B and some C.

1 Inadequate planning. 4 4 16 4 Prior to anchoring, an anchor plan should be developed by the Master 3 2 6 B
and communicated effectively to the involved Officers. In order to prepare
the anchoring plan, the following should be taken into account: the
duration of stay at the anchoring position, information from the Pilot
books, Nautical Chart, the Guide to Port Entry, weather reports, Navtex.
The Plan should include at least the following: anchoring position; speed
and course of approach; which anchor to be dropped; length of cable to
be lowered; method of anchoring; distance from other vessels
2 Defective anchoring equipment. 4 4 16 4 The length of chain that should be lowered should be calculated taking 4 1 4 B
into account sea depth, quality of the sea bed, ship’s size, loaded/ballast
condition, weather conditions, prevailing current conditions, sea space
radius available and duration of the stay at the anchorage. At least a safe
distance of 5 cables from other vessels in the vicinity should be ensured
during the entire anchoring period.
3 Improper windlass operation. 4 4 16 4  . Use of Personal Protective Clothing and Equipment (Integrated 4 1 4 B
Inadequate anchor chain monitoring. Management System).
Adverse weather/sea/current condition.  OCIMF “Anchoring Systems and Procedures for Large Tankers”.
Dangerous sea bottom.

Darkness/inadequate illumination.  Company’s Emergency Response procedures.

 Close contact with port authorities and local agents.

 “Checklist before vessel’s departure”.

 Continuous anchor watch

4 Injury of Ship’s personnel during 4 4 16 4  All personnel involved in anchoring operation to be equipped 4 1 4 B
anchoring.
with the appropriate PPE such as dust mask, goggles, gloves,
safety helmet, safety shoes, overalls etc.

 Master should ensure availability of all data and information

necessary for pre-planning the anchoring operation and that
such information has been adequately communicated to the
involved crew, who should be instructed accordingly. Safety
meeting to be contacted. To be participated all involved crew
5 Damage to the Anchoring equipment. 4 4 16 4  The anchoring operation to be conducted by walk out the 4 1 4 B
anchor’s chain till the anchor touch the sea bottom, in order to
avoid any excessive load and subsequent wear to the windlass
and its associate equipment.

 Vessel’s engine to be used as well for avoidance of excessive

tightness of the anchor’s chain and the subsequent forces to the
anchoring equipment
6 Loss of anchor 4 4 16 4  The anchoring operation to be conducted by walk out the 3 1 3 C
anchor’s chain till the anchor touch the sea bottom, in order to
avoid any excessive load and subsequent wear to the windlass
and its associate equipment
7 Dragging of anchor 4 4 16 4  Weather report to be close monitored and detailed examined. In 3 1 3 C
case of a bad weather is predicted, the anchor to be heaved up
on time.
 the quality of sea bottom to be carefully examined
8 Slip & fall on wet deck 4 4 16 4  Anti-slip paint to be applied to all mooring/anchoring areas. 1 3 3 C

 All personnel involved in anchoring operation to be equipped

with the appropriate PPE.
