PTW-06 Electrical Work Permit

This document is an electrical work permit that outlines safety procedures for electrical work. It requires identifying the scope of work, location, equipment involved, isolating points, locking out energy sources, conducting tests to ensure isolation, informing others, and appointing an isolating authority and team leader. Upon completion, it must be signed by the isolating authority and team leader before restoring power and removing lockouts. The permit aims to prevent electric shock, machinery start-up, and other hazards during electrical work.
