age, Filipinos and residents of Lucena City, after having been duly sworn to in
accordance with law hereby depose and state:
That we personally know MARIA CHRISTINA PERALTA
DAYCO;
That we know the true and correct spelling of the above-
mentioned name, we know he been using since childhood and all
her documents;
That however, she found out in Transfer Certificate of Title
No. T-44570 her name appeared as MA. CHRISTINA PERALTA
DAYCO, hereto attached;
That we hereby attest that the names the MARIA
CHRISTINA PERALTA DAYCO and MA. CHRISTINA PERALTA
DAYCO refer are but one and the same person;
That we are executing this affidavit to attest to the truth of the

SPECIAL POWER OF ATTORNEY
KNOW ALL MEN BY THESE PRESENTS:
NINA RIA AMAZONA VILLAREAL, of legal age, Filipino and resident of Lucena City, have
made, constituted and appointed, and BY THESE PRESENTS do make, constitute and appoint
PAMELA I. ALCARIO, of legal age, Filipino and a resident of Lucena City, to be my true and lawful
attorney-in-fact, for myself, in my, place and stead, to perform the following specific act, to wit:
1. “To appear and transact in my behalf at BUREAU OF INTERNAL REVENUE,
ASSESSOR’S OFFICE, TREASURER’S OFFICE, REGISTRY OF DEEDS and/or
other government or private offices, to facilitate the processing of whatever
documents necessary to transfer of Title covered by Transfer Certificate of Title No.
T-44570 located at Brgy. Gulang-Gulang, Lucena City;
2. To verify and to secure copy of Tax Declaration, Tax Clearance and Certifications
from Municipal/Provincial Assessor’s Office and Municipal/City/ Provincial
Treasurer’s Office and other Government agency concern of the Property mentioned
above;
3. To sign for me, execute and to submit all pertinent documents that may be necessary
and required by the Bureau of Internal Revenue, relative to the payment of the
necessary taxes and to obtain Certificate Authorizing Registration (CAR) of the
subject property and verified my TIN Number;
4. To sign for me, execute and to submit all documents that may be necessary and
required by the Register of Deeds and Provincial/Municipal/City Assessor’s Office
and pay the cost and required fee in processing the annotation/transfer of ownership
and receive/secure new Certificate of titles and Tax Declaration in my favor.
HEREBY GIVING AND GRANTING unto my said attorney full power and authority to do and
perform all and every act and thing whatsoever requisite and necessary to be done in and about the
premises, as fully, to intents and purposes, as we are might or could do if personally present, with all
power of substitution and revocation, and hereby ratifying and confirming all that our said attorney or
their substitutes shall lawfully do or cause to be done by virtue hereof.
