Canteen Services Satisfaction Survey

This survey questionnaire aims to assess the level of satisfaction of senior high school students with the canteen services at Asian Pacific Christian School Incorporated. The survey contains questions in four sections that evaluate experiences with cost, menu, facilities, and staff/crew at the school canteen. SURVEY QUESTIONNAIRE

This study will be conducted at Asian Pacific Christian School Incorporated,

entitled Level of Satisfaction of Senior High School Students in Canteen

Services that aims to know the satisfaction level of the students towards the

school canteen services. The researcher will surely value your participation in

helping them to accomplish the goal of this study.

Direction: The questionnaires will only focus in knowing your satisfaction level

towards the school canteen services there are choices below where you can

select the satisfaction level of your choice. To complete this survey, you are

requested to mark a (check) in every form below for your answer. Please take

time reading the questions carefully and do not leave any information blank

unless stated optional.
