MEDICAL FITNESS CONFIRMATION

I am fit for seagoing duty for the proposed rank onboard and there has been no change in my health condition after Pre-Employment Medical Examination (PEME).

I have never made or received settlement of a medical disability claim. Please declare in writing if you have made any disability claim or have received settlement.

CONFIRMATION OF THE AGREEMENT

By signing below, seafarer confirms that he has read & understood applicable Collective Bargaining Agreement (CBA) and Terms of Employment (TOE), providing detail of
other terms , including repatriation, termination, Health and social security benefits, injury / sickness wages, death & disability compensation etc.
The current and applicable CBA & TOE shall be considered incorporated into and form part of this agreement , which will be binding on both parties.
Signing this agreement binds the Seafarer to sign the appropriate articles of the ship, assigned from time to time by the Ship Owner.
Seafarers confirms receipt of Complaints handling procedure and various company policies including Privacy policy.
Any dispute or difference arising out of or in connection to this Agreement , shall be referred exclusively to arbitration at the port of engagement.

I agree that possession, use and trafficking of Drugs and breach of the Company’s Drug and Alcohol Policy will be just cause for termination of
my employment with my repatriation and my reliever’s joining expenses at my cost

I agree to co-operate fully with U.S Drug Enforcement Agency or any other agency, with regards to implementation of provisions contained in
the Sea Carrier security Manual.

There is no legal case and visa restriction, which restricts from joining a ship

I have not been charged any recruitment fee and not promised any extra benefits and facilities such as shorter contract etc, other than what is
mentioned in the contract

150hrs/month for catering department.

Company provides Medical attendance, it should be used cautiously.
