What is the primary responsibility of HSE officer at site?
Carried out site inspection and ensure complying HSE plan requirement at site.
Verified and ensure PTW system and related doc. complying for site activities.
Follow reporting system such as site observation, near miss, incident reporting etc.
Advise to workforce important of HSE requirement.
Conducting enhancing program for work force to improving attitude towards HSE such as
weekly Safety moment, emergency evacuation plan, participate to conduction mock drill and
all HSE related training.
Make sure availability of facilities arrangement system.
Ensure to keep HSE related required doc. (valid 3 rd party certificate, insurance, ST dept.
inspection check list) at site.
Conduct inspection and maintain check list for FBH, FE, smoke detector, etc.
Keep on continues monitoring to site activities to avoid unwanted event.
Participate and present TBT to work force.

1. Excavation hazardous and control measures?

2. Explain
Fatality (FAT)
A fatality is a loss of life (death) resulting from a Work Injury, or Occupational Illness, regardless of
the time intervening between injury/illness and death.
First Aid Case (FAC)
Any one-time treatment and subsequent observation of minor scratches, cuts, burns, splinters, etc.,
which do not ordinarily require medical care by a physician. Such treatment and observation are
considered First Aid even if provided by a physician or registered professional personnel.
Lost Time Injury (LTI)
A fatality or lost work day case. The number of LTI’s is the sum of fatalities and lost work day cases
(LTI’s = fatalities + lost work day cases). Note: if, in a single Incident 20 people receive lost time
injuries, then it is accounted as 20 LTI's (not 1 LTI).
Lost Workday Case (LWC)
Any work-related injury, other than a fatal injury, which results in a person being unfit for work on
any day after the day of occurrence of the occupational injury. “Any day” includes rest days,
weekend days, leave days, public holidays or days after ceasing employment. (Note: This definition
deviates from that of OSHA which considers restricted work as a lost workday case).
Medical Treatment Case (MTC)
A Medical Treatment Case is any reportable injury that involves neither Lost Workdays nor
Restricted Workdays but which requires treatment by, or under the specific order of, a physician or
could be considered as being in the province of a physician. These are Cases that are not severe
enough to be reported as fatalities or lost work day cases or restricted work day cases but are more
severe than requiring simple first aid treatment.
Near Miss
An unplanned or uncontrolled event or chain of events that has not resulted in recordable injury or
physical damage or environmental damage but had the potential to do so in other circumstances. It
is an incident which could have but did not result in injury, illness, damage, product loss or harm to
the company reputation.
Restricted Workday Case (RWC)
Any work-related injury other than a fatality or lost work day case which results in a person being
unfit for full performance of the regular job on any day after the occupational injury. Work
performed might be: a) an assignment to a temporary job; b) part-time work at the regular job; c)
working full-time in the regular job but not performing all the usual duties of the job. Where no
meaningful restricted work is being performed, the incident should be recorded as a Lost Workday
Case.
Lost Time Injury Frequency (LTIF)
Number of Lost Time Injuries per million hours worked.
