ANNUAL MEDICAL EXAMINATION REQUIREMENT Laboratory Tests Requirement / Issuance of Medical Certificate

1. For the students to facilitate their laboratory tests, attached with this memo is a copy of the
request that they’ll need to present to the medical laboratory of their choice. Colleges are
requested to provide copy of the request to the students.

2. Physical examination and issuance of Medical Certificate will be on a “first come, first serve
COLLEGE OF EDUCATION, ARTS AND SCIENCES basis.” The Health Services Unit will only accommodate 100 students per day.

Laboratory examination: 3. Incoming 2nd-4th Year Students are required to do their Annual Medical Examination on the dates
indicated below. While Freshmen applicants will be accommodated on any day, and still on a
a. Complete Blood Count (CBC) and Blood typing (freshmen student) “first come, first serve basis.”

4. On the Physical Examination day, students must bring all original results of their laboratory tests
c. Chest Xray (PA view) (an updated result or at least done within 2 months period), 1x1 picture (for freshmen and
d. COVID-19 Vaccination Card transferees and have your physical/medical exam with our College Physician. STUDENTS WITH
NO LABORATORY TEST RESULTS WILL NOT BE ACCOMMODATED.
*Note: for working students who had their physical done for the past 2 months,
a certified true copy of laboratory and x-rays are required / accepted.

5. After the physical exam, the College Physician / College Nurse will issue a Medical Certificate.
Gerald S. Bernal, MD The student will be issued 3 copies of Medical Certificate, Student’s copy (student’s personal
College Physician copy), Coordinator’s copy (to be submitted to Program Coordinator) and Registrar’s copy
(to be submitted to the college Registrar).

6. Those students who will have their On-the-Job-Training and will have their Medical Examination
during the Mid-Year term, do not need to repeat their Medical Examination for 1st Semester,
Academic Year 2023-2024.

7. For students who will have their laboratory tests at James L Gordon Memorial Hospital, the
hospital will only accommodate 100 students per day, from Monday to Friday 8:00 AM –
11:00 AM ONLY.
