RISK, SAFETY, AND AWARENESS FORM

(Community 'Kapon' Event)

BLOOD TEST
Before any surgery, a mandatory blood test is a requirement. This essential measure
assesses your pet's overall health and ensures that your pet is fit for the procedure. While it may
not comprehensively screen overall health status, the blood test can prevent potential
complications arising from underlying health issues post-surgery. You must review and
sign a waiver form if you opt out of the blood test.

VACCINATION AND HEALTH SCREENING TEST

Participating in a community 'kapon' event exposes your pet to potential contact with other
animals, pet cages, and carriers during travel, registration, and at the main venue. Owned pets
and strays may be present, heightening the risk of disease transmission. Despite our team's
adherence to high hygiene and biosecurity protocols, events can be unpredictable. Therefore,
protecting your pet through vaccination and screening tests is a crucial responsibility for pet
parents.

Vaccination is essential to shield pets from deadly and highly contagious diseases transmitted
from other animals or the environment; it is necessary to administer vaccinations. Health
screening using test kits ensures that pets are free from detectable diseases, which could
manifest after surgery and pose severe challenges during recovery. Both vaccination and health
screening are imperative, akin to blood tests, to ensure a smooth and secure 'kapon' experience
for your pet. If you choose not to have your pet vaccinated or screened/tested before the
event, you must comprehend and sign a waiver form.

ANAPHYLACTIC SHOCK AND ALLERGIES

Our team is capable of handling rare instances of organ shock or collapse due to vaccines,
injectable medication, or anesthesia. While such complications are infrequent, it's essential to
acknowledge the possibility of severe reactions that could be fatal. Our mortality rate is
0.003% or 27 cases out of 70,000+ patients (as of December 2024).

SPAY-TERMINATION (ABORT) PROCEDURE

Cats are known for prolific breeding capabilities, with pregnancies lasting 62 days. They can
resume mating a month after giving birth, often resulting in an average of 3 kittens. In light of
this, it's not uncommon for us to encounter pregnant cats that initially seemed suitable for a
regular spaying procedure. While some pet parents anticipate this possibility, others may feel
surprised and inclined to question the decision to proceed with termination-spay.

We highly value and respect the perspectives of pet parents in this matter, offering a full refund
if they choose not to proceed with the spay-termination procedure for their pet. However, it's
crucial to understand that in cases where pregnancy isn't detectable through palpation and the
cat is found pregnant during surgery, we must proceed with the termination spay. Pet parents
will be responsible for the additional fees and medications incurred in such cases.

POST-SURGERY COMPLICATIONS AND ILLNESS

Our commitment extends beyond the service to cover post-surgery complications or illnesses,
provided the pet parent presents all necessary tests and examinations conducted before the
event. Failure to do so necessitates the pet parent covering any resulting medical costs or
hospitalization.

POST-OPERATIVE CARE AND EMERGENCY

Recovery from anesthesia varies, and although our team ensures stable vitals and offers
guidance, we expect our beloved pet parents to inform the team promptly in rare
emergencies and, if deemed necessary by our veterinarian, transport the pet to a nearby
clinic

OUTSIDE CLINIC SET-UP

As a home-service veterinary team, our equipment is limited compared to clinics, but we have
the basics to deliver services and respond to emergencies. When extra equipment is needed,
we may refer you to nearby clinics for further assistance.

ANIMAL ESCAPE INCIDENTS

In open-space service events, pets may attempt to escape. Pet parents are responsible for
securing their pets in cages, and the team is not liable for any search and rescue efforts.

WOUND MANAGEMENT & REPAIR

True to our commitment, our service extends beyond the initial treatment phase, as we continue
to support our clients during their pets' recovery period. Nevertheless, pet parents play a
crucial role in ensuring their pets' swift and successful recovery by utilizing suitable
cones or recovery suits. Should issues such as wound licking, biting, or excessive movement
arise, we expect the pet parents to promptly inform the team and follow the instructions provided
by our veterinarian. The pet parent is responsible for any expenses incurred for such issues if
addressed at another veterinary clinic. The healing duration may range from 1-2 weeks or more,
contingent on the pet's behavior, management, and the commitment of the parent to oversee
wound care and administer necessary medications.

Issues about the wound that happened more than (2) two days without any update will be
considered negligence by the pet parent, and the team will not be required to give abrupt
assistance. The pet's parent is responsible for bringing the pet to the nearest veterinary
clinic.

VACCINATION REACTIONS AND APPOINTMENTS

Pets may experience vaccination reactions, showing signs of weakness and loss of appetite
within 24 hours of vaccination. While this reaction is considered normal, it demands additional
care from the pet parent. Although we make efforts to assess the pet's suitability for vaccination
thoroughly, the pet parent is also expected to promptly take the pet to any nearby clinic if
the reaction persists beyond 24 hours and our team is not readily available.

Furthermore, vaccinations adhere to specific schedules, and it is the responsibility of the pet
parent to secure another appointment for the continuous or complete administration of vaccines
and other wellness programs. While we do not assume the responsibility of reminding pet
parents, our team will try to assist in this regard.

POST-SURGERY BEHAVIORAL CHANGES

Typically, neutering contributes to managing behavioral concerns in pets, such as aggression,
spraying, and roaming outdoors. Nevertheless, pet parents should recognize that this is not
universally applicable. Some pets may exhibit contrasting results or show no
improvement due to ingrained behaviors or habits that are not rectifiable through neutering.

STRAY ANIMALS & TNVR PROGRAMS

We have witnessed diverse outcomes in our collaboration with animal rescuers for
Trap-Neuter-Vaccinate-Release (TNVR) programs spanning over two years. As a veterinary
team that values the lives of animals irrespective of their breed, we emphasize the importance
to animal rescuers and pet parents of refraining from immediately releasing strays after surgery
unless they are fully recovered or healed. It is highly advised that strays wear recovery cones or
suits to prevent biting and licking of wounds. Furthermore, it's essential to bear in mind that the
recovery process for stray animals can sometimes extend beyond the usual duration due to
potential underlying health issues prevalent among them. Therefore, a more stringent and
regular observation is imperative post-surgery.

Those bringing strays to our event must be capable of housing them until full recovery,
as we are not liable for sudden releases impacting wound recovery.

POST-SURGERY MODIFICATIONS AND ADJUSTMENTS

Over the past three years, our team has successfully neutered more than 50,000 pets, earning
organic and authentic high recommendations and positive online reviews. These accolades
underscore our commitment to excellence in veterinary services.

In rare instances where the anticipated outcome of the surgery, such as persistent heat cycles
after spaying, is not achieved, it is essential to note that various factors may contribute. These
factors include late organ development, compensation mechanisms, tumors, and abnormal
hormonal supply from other organs. Requests for corrections, adjustments, or related
treatments will be subject to a thorough evaluation through requests and examinations.

I read and understand all the statements above related to the risk and safety of the
services, procedures, and events. As the parent/authorized guardian of the animal, I am
taking full responsibility. I will not hold anyone liable for any unexpected incident that
may happen that can't be under anyone's control. I also agree to not post any derogatory
statement on any social media to anyone, especially the team, for whatever possible
outcome of the service requested. I am made aware of the legal consequences of not
following the agreement in this form.
