INTRODUCTION: - Client was brought in psychiatric OPD with the complaint of over
talking,unnecessary laugh and increase in motor activity.

GENERAL APPEARANCE:

Client tells the similarity between both fruits,

Dissimilarities between orange apple saying just he saying that orange is covered & apple is not covered.

a) Arithmetic ability: client . Can add, subtract and multiply simple equations.

b) General knowledge: inadequate general knowledge (client having no interest in general

knowledge)

JUDGEMENT

Social judgment: impaired

Sample:- When client asked to “ what would you do if you found letter in road”.

He replied “I will not touch the letter ”

Personal judgment: intact

Sample: When client asked to “what is your future plan”

He replied “After going he will do some work”. He knows that he is next to his father to run the family.

Test judgment:-

INSIGHT

Grade: III awareness of being sick attributed it to it to external or physical factors

CONCLUSION

After getting the history, investigation & physical assessment of patient. I conclude

that patient is bipolar disorder. I have given the proper nursing management related to

his problem. Patient was satisfied & honestly solve his double related his problems.
