POLICY FOR PATIENT MANAGEMENT BASED
ON ARD GUIDELINES
POLICY STATEMENT
Misamis Oriental Provincial Hospital Manticao is committed to providing appropriate and timely care to
patients through the Admit, Refer, Discharge (ARD) process. This policy outlines the guidelines and
procedures for admitting patients, making referrals when necessary, and ensuring a smooth and safe
discharge process.
POLICY GUIDELINES
ADMIT
a. Admission Criteria: Patients will be admitted based on established admission criteria, which may
include medical necessity, severity of illness, availability of appropriate resources, and compatibility
with the services provided by Misamis Oriental Provincial Hospital Manticao.
b. Admission Procedures: Clear procedures for patient registration, obtaining consent, and
documenting relevant information will be followed. This includes collecting personal and medical
information, verifying insurance coverage, and assigning appropriate identifiers.
c. Patient Safety: Measures will be in place to ensure patient safety upon admission, including
identification of allergies, fall risk assessment, and infection control protocols.
d. Physician’s Call: Patient to be admitted shall be based on the Physician’s medical assessment and
the capability of the patient’s family for home care management. Admission is highly recommended
when patient’s condition is beyond the level for home care management or when the family members
is incapable of providing care to the patient at home.
REFER
a. Need for Referral: If a patient's condition requires specialized care or services beyond the scope
of Misamis Oriental Provincial Hospital Manticao, a timely referral will be made to an appropriate
healthcare provider or facility.
b. Referral Process: The referral process will include communication with the receiving provider or
facility, providing relevant medical information, and coordinating the transfer of the patient's care.
c. Documentation: Accurate and complete documentation of the referral process, including the
reason for referral, communication details, and any relevant instructions, will be maintained in the
patient's medical records.
DISCHARGE
a. Discharge Planning: Discharge planning will begin as early as possible to ensure a smooth
transition from the healthcare facility to the next level of care. The patient's medical stability,
functional status, social support, and post-discharge needs will be assessed to determine the
appropriate discharge plan.
b. Patient Education: Patients and their watchers will receive comprehensive education regarding
post-discharge instructions, including medication management, self-care, follow-up appointments, and
available support services.
c. Discharge Documentation: A final discharge summary will be prepared, including the patient's
diagnosis, treatment provided, medications, follow-up instructions, and any necessary referrals. This
summary will be shared with the patient, watchers, and relevant healthcare providers involved in the
patient's care.
d. Out Patient Care: Patient’s health condition after proper medical assessment who did not qualify
with the required level of medical necessity and severity of illness for admission shall be recognized
as Out Patients and shall be provided with Out patient care management.
