Note: this section can be completed after you have competed your risk assessment.
Outline how the risk assessment was carried To complete risk assessment, I took reference from two preferred web pages [Link] and
out this should include: [Link] where collected relevant information that required for Health and safety in construction
 sources of information consulted; and also noted the recommended action to be taken as per the undertaken work activities.

 who you spoke to; and Having collected all information, it was needed to cross evaluate the process and activities by doing visual
 how you identified: inspection to examine whether the safe code of practice is in place or not or meeting all safety standard
- the hazards; with compliance or not , I then visited workplace, and observed the work process and activities and during
- what is already being done; and my inspection I came across with several unsafe act and unsafe conditions at workplace such as some
- any additional controls/actions that workers were doing their job at work height without wearing appropriate PPEs and working platform seems
may be required. to unstable since inspections was last carried out 2 months ago, seeing all I further spoken to some workers
and their supervisor just to know their knowledge regarding Health and safety and their answer didn’t
convince me appropriately as they said “working at height platform is not more than 10 feet hence we have
been working for 2 months and didn’t encounter with any injuries” their answer made me alert that that
there is lack of safety awareness.

Realising workers action I demanded company last risk assessment, current audit report, work equipment
inspection report, workers health reports, absenteeism record and accident/incident book from HSE
Manager to check if any accidents occurred in past and if occurred , were taken control measure for
preventing such reoccurrence and following accident book checking the type of incidents and frequency of
incidents occurred in past and if any of these incident were recuring and also checked the reason of
incident.

Following accident book, I noted and identified the existing hazard and reviewing all my collected
information from available resource, I could be able to asses workplace process and activities and realize
that these all action required additional control measured required to be adapted.

Part 3: Prioritise 3 actions with justification for the selection
Suggested word counts
Moral, general legal and financial arguments for all actions: 300 to 350 words
For EACH action:
Specific legal arguments: 100 to 150 words
Likelihood AND severity: 75 to 150 words
How effective the action is likely to be in controlling the risk: 100 to 150 words

Moral, general legal and financial arguments for ALL actions

Moral, general legal and financial Moral- Advent construction has also moral obligation to protect their workers by providing a safe and better
arguments workplace, equipment and environment along with competent training and supervision to prevent injuries
from all risk involved in workplace activities is also moral duty of undertaking employer to treat their workers
as their own family member because they work for them for employer growth and in return get small
monitory reward as a wage and workers expect where they work shall be safe and suitable without risk and
expect happy and safely return to home each day and runs their livelihood safely but not to put themselves
failing into risk of injuries but if during prolong work activities any injuries occurred to workers, it will not only
impact on victim health conditions but also impact on witness mental condition that could weaken the
morale of workers and force them to leave the organisation that led bad reputation which will impact on
business .Hence employer shall meet the workers expectation and to provide a safer and better workplace,
equipment and environment to give protection from any risk.
Legal: Failing to meet moral obligation is an act of contravening ILO standard and national laws and legislation
and if injuries occurred due to non-compliance where employer will be responsible for the injuries caused to
workers and in such case undertaking employer will have to face strict legal action against employer where
prohibition notice will be served to employer by the reinforcement agency in the forms of fines and penalties,
will be imposed persecution charges, trade licences will be cancelled, interfere of courts ,could have been
drag to court and subjected to crime , employer may have been sent to jail.
Financial: if any injuries took place, then the cost that incurred in form of direct cost and indirect cost and that
could be categorised in three category such as
1. Cost that incurred on injured workers as a sick pay compensation, hospital treatment
productivity lost, recruiting and training cost of newly joined workers.

Learner number: Learner name: Sample Report 1 Page 22 of 28

2. Cost that incurred on repairing and replacing of damaged equipment, infrastructure.
3. Cost that incurred in following accident investigation by engaging key personnel,
enforcement action costs and several legal costs and duties.
Apart from above incurred costs, some costs are insured and recoverable but there are some uninsured costs
that can’t be calculably returned such as OT cost for balancing down time, emergency supply cost, trained
person lost, loss of reputation, loss of morale, loss of contract etc that all will be devastating for undertaking
company’s finance.
Keeping all moral, legal and financial argument employer must ensure to meet all obligation to avoid bad
consequence that could fall on undertaking company.

Justification for action 1

Action (Taken from column 4 of risk “Advised to provide inspected PPEs such as safety helmet, boot, gloves, jacket etc including FBH in sufficient
assessment) number to all engaged workers ensuring its use.”

HAZARD CATEGORY-WORK AT HEIGHT.

Specific legal arguments C167 - Safety and Health in Construction Convention, 1988 (No. 167), Article -30

With the reference to above suitable convention where employer are advised to provide suitable and
protective clothing and equipment to protect workers against injury where workers are liable to work in
adverse conditions realising the type of work and its risk , its employer duty to provide such protective
clothing without cost to workers and shall train workers for the use of protective equipment individually and
ensure its proper use and maintenance as prescribed by country’s laws and legislation whereas workers shall
also use properly and take good care of provided protective clothing and equipment, but such suitable
protective clothing and protective equipment are not provided by the undertaking employer which is act of
contravention of ILO and prescribed country’s laws and legislation and during prolong risk activities any injuries
occurs caused harm to workers due to non-compliance of ILO standard , in such circumstances , undertaking
employer will be accountable for injuries caused to workers where a series of strict legal action will be taken
against him such as imposing several charges, legal duties will be imposed which could hamper ongoing
projects and upcoming projects and ultimately have to close the projects.
Consideration of likelihood AND severity The work at height activities has always possess high risk of injuries and here in advent construction most of
the activities are also being carried out at work at height where working platform is unprotected and
uninspected where there is High likelihood of falling of person from working at height platform either due to

Learner number: Learner name: Sample Report 1 Page 23 of 28

open edge or collapse of working platform and since PPEs are also not worn by the engaged workers where
there is Quite Very High likelihood of causing injuries to fallen workers and person below the working
platform.
The severity of injuries depend on what height workers falls and how intensely hitting by the objects such as
 types of injury or ill health cuts/bruise/body parts fracture/severe head and spinal injuries/disability and in worse case death occur.
43 Workers are at risk
 number of workers at risk
 how often the activity is carried The work at height activities is being carried out on an average of 7hrs/job/workers on daily basis for all
out different location in 2 shifts.
 how widespread the risk is
The risk is concentred within work at height premises and its circumference.

How effective the action is likely to be in

My advised action is likely to be controlling the risk by providing the suitable protective clothing ensuring it
controlling the risk. This should include:
must wear while working at height which will protect workers from injuries and will improve the safety of
 the intended impact of the action; workplace and also ensure the safety of all engaged workmen.

 justification for the timescale that I have given 1 week to complete action because only it required to check what number to be purchased and
you indicated in your risk after undergoing official formalities and after getting purchase approval, could be easily purchased from
assessment; and approved vendor which is now a days is easy to find. Management must not take more than 1 Week realising
high likelihood and catastrophic result. Hence 1 week is sufficient enough to complete the action.
 whether you think the action will My advised action will fully control the risk by wearing provided suitable protective clothing such as FBH
fully control the risk ensuring its 100% tie off with anchorage point with strong hook stability that will ensure to not let the workers
falls and control the risk of injuries, in addition to this other recommended control measures as advised must
be in place in right direction within given time scale.

Justification for action 2

Action (Taken from column 4 of risk “Heat, smoke and fire detection devices along with fire alarm system must be installed to detect fire at its initial
assessment) stage to control the risk of fire outbreak and to minimise the injuries”.

measure to avoid the risk of fire and shall take all required precautionary arrangement so to combat quickly
and efficiently any outbreak of fire and its also employer duty to bring about a quick and safe evacuation of
persons but in right now undertaking employer is not installed Heat, smoke and fire detection devices along
with fire alarm system to detect fire at its initial stage to control the risk of fire outbreak and to minimise the
injuries where employer neglects the compliance of standard ILO convention and breaching the ILO act and in
case of any injuries that occurred due to prolong high risk activities of hot work activities and causes injuries
to workers in such circumstances undertaking employer will be accountable for the injuries caused to worker
where employer will be treated as contravener and inconsequence , employer will have to face a series of
strict legal action against him that could series impact on its reputation, could be imposed several legal and
enforcement action and even punish for the non-compliance negligence and will hit a serious impact on
running projects. Hence its employer duty to meet all standard and fix the issue to avoid such bad
consequences.
There is High likelihood of fire occurrence as hot work activities is being carried out in enclosed room (where
Consideration of likelihood AND severity
no proper ventilation system is installed) with the maximum use of electrical tools and equipment and use of
argon gas cylinder in presence of flammable material at workplace and the flame and spark emission through
the activities either due to faulty equipment or short circuit has Quite Very High likelihood of catching and
spreading of fire could cause several personal injuries.

 types of injury or ill health Severity of injuries occurred to workers depending on the what level of fire and duration of fire exposed to
workers such as burns, asphyxiation due to prolong exposure of toxic gas, smoke and fumes and could be
worse if person can’t be evacuated in time could occur death and moreover if fire contaminates with other
accumulated flammable and hazardous substances turn the fire into fire explosion which could be
catastrophic results for property and environment.

 number of workers at risk Primarily 21 proactive engaged workers at high risk such as welder, grinder man, driller and electrical
supervisor etc but other person such as lone workers, disabled person and office employee are also at risk.
 how often the activity is carried The Hot work and electrical work activities are being frequently carried out on an average of daily basis

controlling the risk. This should include: devices along with interlinked fire alarm system which will improve the workplace safety and ensure the
safety of all engaged workers working at workplace.
 the intended impact of the action;
I have given 2-week time scale to complete releasing its high likelihood and catastrophic results occurrence

assessment; and from selected vendor and after its inspection can be installed by competent person who is available at
workplace. Hence all these processes shall be completed within 2 weeks easily.
 whether you think the action will My advised action will fully control the risk of fire occurrence by installing Heat, smoke and fire detection
fully control the risk devices along with fire alarm system because the functions of heat, smoke and fire detection devices is to
detect fire at its incipient stage and with installed interlinked fire alarm system that helps to alert any fire
outbreak and noticing such alarm fire could be easily extinguish with help of fire fighter by the installed fire
extinguisher and will not let fire to be caught and spread , in conjunction with these other recommended
control measure shall be in place in right direction withing their given time scale.

HAZARD CATEGORY-MANUAL HANDLING
Specific legal arguments C127 - Maximum Weight Convention, 1967 (No. 127), Article- 3
R128 - Maximum Weight Recommendation, 1967 (No. 128), VI.
Maximum Weight in Section A –Adult Male Workers.

In Undertaking company manual handling operation is done, where heavy load containing more than 25kg
and possessing hazardous material by individual is being carried out where likely to affect workers’ health by
means of its weight where employer is breaching above mentioned ILO conventions which specifically states
that no worker shall be engaged to manual handling or load transporting by means of its weight, workers likely
to jeopardize workers’ health or safety and during manual handling operation and also breaching countries
laws and legislation and if workers suffer from any injuries due to heavy load where employer is accountable
for injuries caused to workers due to non-compliance of ILO act and in such case employer will have to face
legal consequences that could serious hit on its business.
Consideration of likelihood AND severity Engaged loaders are carrying heavy load that contain pointed material, hot iron, glass material that possess
more than 25 kg weight for an extended time period without taking rest break, moreover manual handling

 whether you think the action will My advised control measures will fully control the risk by assigning competent person because assigned person
fully control the risk will ensure that the maximum load is under controlled and maintained, ensuring no worker shall be engaged to
manual handling or load transporting by means of its weight, workers likely to jeopardize workers’ health or
safety and will not let cause injuries from manual handling operations, along with other control measures
shall be in place and effectively in compliance.

Part 4: Review, communicate and check

Suggested word counts for each section:
 Planned review date or period and reasoning for this: 50 - 100 words
 How the risk assessment findings will be communicated and who needs to know the information: 100 - 150 words

reasoning Therefore, I planned review date on 3rd November, 2024, under the following circumstances
4. If there is no change in working pattern and schedule
5. If there is no change in brought in work activities and process
6. If there is change in work equipment.
7. Upon introducing new substances and technology
8. If there is glitch in implementing HSE policy
9. If there is incident revealing hazards.
How the risk assessment findings will be To communicate my risk finding of completed risk assessment, I will first of all arrange a meeting and will
communicated AND who you need to tell invite all responsible person such as project manager, project engineer, HSE Manager and member from all
department and will discuss on my risk finding and get them agreed to implement my recommended control
measure within time scale. I will also email a soft copy to almost all department and safety committee
member team requesting to consider my risk assessment finding in Company upcoming TBT.I will also display
my risk finding on company notice board with proper warning sign so that workers could notice. I will also
meet all workers personally in their rest break time counselling workers to take precaution until the
recommended action is to be implemented.
How you will follow up on the risk Having communicated my risk finding, I will set reminder in my mobile and also note down in my dairy of all
assessment to check that the actions minutes of meeting and will be in touch with all of the responsible person whom I assigned the responsibility
have been carried out and when reaching closer to minimum time scale given. I will ask for the latest update for the action to be
taken and while on talking on mobile if I will find any glitch or discrepancies in their statement .I will plan to
revisit the workplace and will meet all concerned person and will ask the reason of delayed caused to
implement and would like to know the reason whether its lack of resources, manpower or any financial
concern .I will also meet the project manager who has responsibility for managing health and safety at work
place and request to fix the issues and I will also meet the Director along with project manager briefing all
hurdles encountering implanting issues and will request to director to solve immediately realising the bad
consequences of noncompliance of risk finding which could impact on business because it is act of
contravening ILO act.
