TO WHOM IT MAY CONCERN:

We would like to inform you that consent is hereby granted to our son/daughter, <name
of the student> to undergo in the Off-Campus Activities of both curricular and non-
curricular activities that are necessary for our child to have a more meaningful learning
experience aside from their regular classroom instructional activities and necessary to
their degree program. Moreover, we do also believe that through these various types of
off-campus activities it broaden the students’ learning and learning opportunities and
feel the real world.

With all our consent, we are allowing our child to participate to the different off-campus
activities that are designed by their teachers as part of their requirement of the course
(PLEASE WRITE HERE ALL THE COURSES IN THE 2 ND SEMESTER), and the
activities outside to be conducted by the CORDS, that will cover within Second
Semester of the School Year 2024-2025.

We are aware that the Personnel In-Charge/s will take all necessary precautions to
keep our son/daughter safety. Likewise, the Program Chair will also do regular
monitoring on the conduct of the off-campus activities and monitor the performance and
safety of our son/daughter.

However, if despite all the efforts taken, untoward incidents beyond their control occur
we will not hold Northwest Samar State University as well as the community where our
son/daughter took the off-campus activities accountable.
