LORENA C. ERIVE, of legal age, Filipino, married, and with residence at Brgy. San Miguel Tarlac City, Philippines, NESTOR B.

name, constitute and appointed, SALDY SEQUIJOR LOPEZ, of legal age, Filipino citizen, and with residence at Brgy., San Isidro,
San Luis, Aurora, as true and lawful attorney in-fact to act, for and in my stead, to do and perform the following acts deeds and things,
to wit:
1. To cause the transfer of registration of parcel of land, located at Barrio of Quirino,Municipality of
Maria,Aurora,Province covered by TRANSFER CERTIFICATE OF TITLE NO.: 051-2012001615
2. To claim and receive the Certificate Authorizing Registration from the Bureau of Internal Revenue (BIR);
3. To transact business with the Bureau of Internal Revenue (BIR) including TIN applications of the necessary
parties if no available in record, Assessor’s Office (Tax Declaration, No Improvements) Registry of Deeds, Land
Registration Authority, Department of Agrarian Reform (DAR) INCLUDING application for Clearance,
Affidavits and other necessary documents, Philippine Statistic Authority (PSA), marriage certificate, Birth
Certificate and marriage certificate, and other government offices in all matters relating to the registration of said
property in the name of the DONEE;
4. To make, sign and execute, for and in my behalf, any documents which may be necessary for the release of the
certificate of title in the name of DONOR ;
5. To sign, execute, receive and/or deliver contracts and agreements, other papers and/or money to effect with his
power stated in this authorization pertinent to the above-described authority.
6. To do such and other act or thing that may be required necessary or incidental to carry out effectively any and all
of the purposes for which this authority is hereby given; HEREBY GIVING AND GRANTING unto our said
attorney-in-fact full power and authority to do and perform any and every act, and thing whatsoever requisite and
necessary to be done in and about the premises, as fully to all intents and purposes as I might or could lawfully do
if personally present and hereby ratifying and confirming all that my said attorney-in-fact shall lawfully do our
cause to be done by virtue of this presents.
