Respected sir,

Iam S.VENKATA SIVA PRASAD,SSE/BOGIE POH/CF/CRS/TPTY would like to represent my


explanation on the charges that During CWM/TPYS shop inspection on 23-1-2025, dust lots not
removed on the shop floor after cleaning.

In this context I would like to explain that house keeping staff after grooming of shop floor dust
lots were placed in the middle of the pathway which affected the movements of working employees. As
soon I was noticed the dust lots I instructed the house keeping staff to remove the dust lots
immediately and they started clearing the path way but it was taken more time to clear the dust.

Hence I request to kindly accept this as my last mistake and it will not be repeated in future.
