PROCEDURE CHECKLIST
PHOTOTHERAPY

DEFINITION:
Phototherapy is the application of fluorescent lights over the infant's skin to assist
in reducing the serum bilirubin level in the infant's blood.

PURPOSES:
 To support the care of babies with hyperbilirubinemia.
 To decrease infant bilirubin levels.
 To maintain phototherapy treatment safely and effectively.
 To minimize maternal-infant separation and facilitate breastfeeding.

PROCEDURE 5 4 3 2 1 REMARKS
1. Perform hand hygiene and observe other appropriate
infection-prevention procedures.
2. Place the newborn under a radiant warmer or in an
isolette under phototherapy lights so that it can be kept
warm.
3. Position the phototherapy lamp based on unit protocol
and the type of lights used to allow effective use of the
phototherapy unit. Place the lights 12 to 30 inches
above the newborn's bassinet or incubator.
4. Undress the newborn so that as much light as possible
can reach the skin. Put a small diaper or a face mask
to cover the genitals.
5. Place eye shields (opaque) over the infant's eyes at all
times during phototherapy as a prophylaxis and to
protect against retinal damage that can occur due to
irradiance from phototherapy light. Ensure the infant’s
eyes are closed when positioning the eye shields to
prevent corneal abrasions.
6. Monitor and record the newborns temperature at least
4 hourly or more frequently as clinically indicated to
ensure the infant does not become cold stressed,
hypothermic or hyperthermic. Observe skin
temperature by “warm to touch” method or by
peripheral skin probe measurement.
7. Remove the newborn out from under the lights for
feedings and cuddling.
8. Monitor the newborn’s intake and output closely.
9. Observe for frequent loose green stools and dark-
colored urine.
10. Cleanse skin thoroughly and change nappies when
soiled to prevent nappy rash and maintain skin
integrity.
11. Weigh the newborn daily to assess fluid loss.
12. Reposition the infant regularly at least every two hours
to prevent skin breakdown.
13. Take blood sample from the heel periodically to check
the amount of bilirubin still in the newborn’s blood.
14. Monitor and record incubator temperature hourly.
15. Document all nursing assessments, nursing care,
serum bilirubin levels in nursing notes as appropriate
to maintain accountability and continuity of care.
