The Hide action allows a player to conceal themselves by succeeding on a DC 15 Dexterity (Stealth) check while being heavily obscured or in cover, and out of enemies' sight. If successful, the player gains the Invisible condition but can lose it by making noise, entering an enemy's line of sight, or performing certain actions. New monsters entering combat may notice the hidden player based on their Passive Perception compared to the player's Stealth roll, and players can attempt to hide again if circumstances change or if they choose to do so.

Rules 5.5e

With the Hide action, you try to conceal yourself. To do so, you must
succeed on a DC 15 Dexterity (Stealth) check while you're Heavily
Obscured or behind Three-Quarters Cover or Total Cover, and you must
be out of any enemy's line of sight; if you can see a creature, you can
discern whether it can see you.

On a successful check, you have the Invisible condition. Make note of


your check's total, which is the DC for a creature to find you with a
Wisdom (Perception) check.

You stop being hidden immediately after any of the following occurs: you
make a sound louder than a whisper, you enter an an enemy's line of
vision, you make an attack roll, or you cast a spell with a Verbal
component.

New Monster Enters Combat

New Monsters' Perception: When new monsters are introduced,


they are not automatically aware of the hidden player.
Passive Perception: The DM will compare the new monsters'
Passive Wisdom (Perception) scores to the player's existing
Stealth roll. If a new monster's Passive Perception is equal to or
higher than the player's Stealth roll, that monster notices the
player. The player is not hidden from that specific monster.
Active Search: If a new monster uses its action to Search for
hidden creatures, it will make an active Wisdom (Perception)
check against the player's existing Stealth roll.
Conditions for Hiding Still Apply: The player must still meet the
conditions for being hidden from the perspective of the new monsters.
If the new monsters enter from a direction where they have a
clear line of sight to the player (and the player isn't, say, invisible
or in total cover from that new angle), then the player is not
hidden from them, regardless of the Stealth roll. You can't hide
from a creature that can see you clearly.
If the player was hidden behind a pillar from Monster A, but
Monster B enters from the side and can see the player, the
player is not hidden from Monster B.
Losing the Hidden Condition: The player remains hidden (from
creatures that haven't spotted them) until:
They make an attack (unless they have a feature that allows
them to remain hidden, like a Gloom Stalker Ranger's Umbral
Sight).
They make a significant noise (e.g., casting a spell with a verbal
component, knocking something over).
They move into a creature's clear line of sight.
A creature's Perception (passive or active) beats their Stealth roll.

When a New Roll Might Be Needed

Changing Circumstances: If the player moves, or the environment


changes in such a way that their previous hiding spot is no longer
viable against the new monsters (e.g., the cover they were using is
destroyed, or the new monsters have a different line of sight).
Player Choice: The player might choose to use their action (or bonus
action, if applicable) to try and Hide again if they feel their current
position is compromised or if they want to try for a better Stealth roll.
DM Adjudication: In very specific, dynamic situations, a DM might
rule that the chaos of new arrivals or a shout from an existing monster
("They're over there!") effectively negates the previous attempt to hide,
prompting a new check or at least giving the new monsters advantage
on their Perception. This is less RAW and more DM discretion.
