Observed no sanitizer setup. A sanitizer solution must be setup at all times food or drink is being
prepared, served, and refreshed at least every 4 hours. Adequate strength for chlorine is 50 ppm.
Adequate strength for QAC is between 200-400 ppm. 228.113(1) 228.114(a)(5)(d). Cleaning of
Equipment and Utensils. Equipment, food-contact surfaces, nonfood-contact surfaces, and

for cold storage. 228.66. Preventing Food and Ingredient Contamination. (a) Packaged and
unpackaged food - separation, packaging, and segregation. (1) Food shall be protected from cross
contamination by: (A) except as specified in subparagraph (A)(iii) of this paragraph, separating
raw animal foods during storage, preparation, holding, and display from:. (i) raw ready-to-eat
food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw
ready-to-eat food such as vegetables; and (ii) cooked ready-to-eat food
Observed foods stored uncovered inside walk-in cooler. Food products, inside of prep coolers,
walk-in coolers and freezers, must be covered with lids or Saran Wrap, to prevent environmental

Observed lived gnats in deli area.
Facility must exterminate for pests until pests have been eliminated. Please
forward next extermination report to health department. 228.186(k)(1-4) (k) Controlling pests.
The presence of insects, rodents, and other pests shall be control to eliminate their presence on
the premises by: (1) routinely inspecting incoming shipments of food and supplies; (2) routinely
inspecting the premises for evidence of pests; (3) using methods, if pests are found, such as
trapping devices or other means of pest control as specified under §228.204(b) and§228.208(b)
and (c) of this title; and (4) eliminating harborage conditions

Observed a defrost fail. Food products must thaw inside of coolers or under running water.
§228.75. Temperature and Time Control. (a) Frozen food. Stored frozen foods shall be
maintained frozen. [38] (b) Time/temperature controlled for safety food, slacking. Frozen
time/temperature control for safety food that is slacked to moderate the temperature shall be
held: [38] (1) under refrigeration that maintains the food temperature at 5 degrees Celsius (41
degrees Fahrenheit) or less; [38] or (2) at any temperature if the food remains frozen. [38] (c)
Thawing. Except as specified in paragraph (4) of this subsection, time/temperature control for
safety (TCS) food shall be thawed: (1) under refrigeration that maintains the food temperature at

Observed the dumpster doors were open. 228.152(f)(1-2),(o) §228.152. Refuse, Recyclables, and
Returnables, Facilities on the Premises. (f) Outside receptacles. (1) Receptacles and waste
handling units for refuse, recyclables, and returnables used with materials containing food
residue and used outside the food establishment shall be designed and constructed to have tight-
fitting lids, doors, or covers. (2) Receptacles and waste handling units for refuse and recyclables
such as an on-site compactor shall be installed so that accumulation of debris and insect and
rodent attraction and harborage are minimized and effective cleaning is facilitated around and, if
the unit is not installed flush with the base pad, under the unit. (o) Using drain plugs. Drains in
receptacles and waste handling units for refuse, recyclables, and returnables shall have drain
plugs in place. NOTE: DUMPSTER LIDS SHALL REMAIN CLOSED WHEN NOT IN USE.
