AFFIDAVIT OF AUTHENTICITY
(CCTV Footage)
I, Francis Agus, of legal age, Filipino, and a resident of Poblacion A,
Borongan City, Eastern Samar, after having been duly sworn to in
accordance with law, hereby depose and state:
1. That I am the [position, parish staff at the Borongan Cathedral, located in
Borongan City, Eastern Samar.
2. That as part of my duties, I am authorized to access and manage the
CCTV surveillance system installed in and around the Cathedral premises.
3. That on February 13, 2025, between 8:00 a.m.to 9:00 a.m.,, the CCTV
camera located inside Borongan Cathedral near the confession area
recorded footage that is relevant to the events described in the case
docketed as Damages Based on Violation of Art.21 of the New Civil Code,
CIVIL CASE No. 182977 pending before the Regional Trial Court Borongan.
4. That the enclosed footage, saved in a USB flash drive is a true, accurate,
and unaltered copy of the recording retrieved from the Cathedral’s CCTV
system.
5. That the retrieval was conducted personally by me using the regular
process of the system, and I hereby certify that no editing, splicing, or
manipulation was made on the said footage.
6. That I am executing this Affidavit to attest to the authenticity and
integrity of the said CCTV footage and for whatever legal purpose it may
serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 29th day of March
2025, in the City of Borongan, Eastern Samar, Philippines.
