:Rheumatoid factors(RF)are a heterogeneous group of autoantibodies reactive with Fc portion of IgG and is found in re thantwo-third of
adults with the disease.Presence of Rf is indicative of Rheumatoid arthritis.However,it is not specific as it also positive in other
inflammatory rheumatic as well as non rheumatic diseases.These include SLE,drug induced lupus,Sjogren syndrome,chronic liver
disease,sarcoidosis,interstitial pulmonary fibrosis,infectious mononucleosis,tuberculosis,SBE etc.Some healthy individuals of over 65yrs
may also have positive results.Despite above limitations,the detection of RF above the referance is a diagnostic criterion of Rheumatiod
arthritis as established by American College of Rheumatology.

COMMENTS:-
1. Cholicalciferol (vitamin D3) is synthesized in the skin from 7 dehydrocholestrol in
response to sunlight. Ergocalciferol (vitamin D2) comes essentially from diet and supplements.
The assay measures both D2 (Ergocalciferol) and D3 (Cholecalciferol) metabolites of vitamin D.
Vitamin D status is best determined by measurement of 25 hydroxy Vitamin D, as it is the major
circulating form and has longer half life (2-3 weeks) than 1,25 Dihydroxy vitamin D (5-8 hrs).
2. Both Cholicalcifecal and Ergocalciferol are converted in liver to 25 OH Vitamin D.
3. 25 OH Vitamin D is considered the best indicator of Vitamin D nutritional status.
4. Vitamin D toxicity is recognized, but is a rare occurance.
5. Vitamin D level shows seasonal variation, with values being 40-50% lower in winter than in summer.
Decreased levels:
* Inadequate exposure to sunlight.
* Dietary deficiency.
* Vitamin D malabsorption.
* Severe Hepatocellular disease.

COMMENTS:-
R.A. Factor determination used widely for diagnosis of rheumatoid arthritis. But it lack specificity
for rheumatoid arthritis. R.A. factor is also positive in SLE, dermatomyositis, cryoglobulinemia &
mixed connective tissue diseases & in some healthy subject as well. However Anti CCP antibody is
highly specific marker for rheumatoid arthritis.
