AUDIOLOGICAL REPORT

Aquino, Demian Denis

CHRONOLOGICAL AGE: 4 years and 9 months

The boy Aquino Demián Denis is undergoing a speech therapy evaluation,


with a frequency of once a week, since September 2021.

The child attends a speech therapy consultation accompanied by his mother via video
call. An exploration is conducted to identify their level of language development, the
articulation of this and based on this information provide guidance in order to address
his need.

Education:

Denis begins his school journey at the age of three at the Nueva Esperanza kindergarten.
Lomas de Zamora. Currently attending a 4-year-old classroom in person.

Cohabiting family structure:

The boy Denis is the only child of the couple formed by Flavia and Nelson, the three
they live in the same house in Santa Catalina, district of Lomas de Zamora.

Significant evolutionary data:

On the first day of the consultation, Denis, along with his mother, undergoes the
initial interview, where he/she reports that he/she tends to throw tantrums at moments when he/she is with
his uncles and grandparents. He likes to play in the square, run, and play with cars which are his.
preference. It also mentions its behavior in the garden, in which
participates in the activities proposed by the teacher. With their classmates, they use
isolated words and non-verbal language. Shows control of sphincters, managing to anticipate
and seek assistance from a third party.

Birth, prenatal, and postnatal history:


The mother refers to being positive for Toxoplasmosis during pregnancy, therefore
reason was medicated.
The pregnancy was full-term with a normal delivery.
Denis was born without any difficulties, he was breastfed until six months of life, he continued
with the use of a bottle until two years old and the use of a pacifier until three years old.

Managed techniques:

Anamnesis.
Behavior observation.
Phonological Tests

Observation of behavior:

Denis is a very sweet boy, he laughs a lot, and at work he shows shyness.
She looks at her mom every time she has to respond.

Upon starting the activities proposed by the therapist, he/she manages to sustain the duration of the
full session. Remains seated.

Recognizes animals and some colors.


A clear communicative intention is observed. He holds eye contact when proposals are made to him.

Recommendations

Speech therapy.

●Guide parents on the activities to be carried out at home to improve language and

It is recommended to continue working on the following objectives:

● Stimulate attentional capacity.
● Accompany the acquisition of learning.
● Perform orofacial practices.
● Work on fine motor skills.
