REASON FOR CONSULTATION

Minor patient, admitted in the company of the mother, who states that
presents learning difficulties

The mother reports that he has difficulties retaining information, he is


forget what you study, there is a deficiency in reading and writing, alteration in the
school skills, in fifth grade.

PATIENT CONDITION AT THE TIME OF THE EXAMINATION

15 sessions have been conducted including the assessment session, it is observed


cooperative patient, in normal general conditions, presents
anxious, partially time-oriented, person and space-oriented,
slow cognitive processing, low tone of voice, does not establish eye contact,
low tolerance to frustration and behavior tending to
impulsivity. Shows difficulty in understanding quickly
medium and high complexity instructions.

Based on the assessment, the difficulties evidenced in Dani are at the level of:

COGNITIVE FUNCTIONS: difficulties in attention periods (speed


processing, sustained and selective attention) concentration, memory
auditory and visuospatial, visual tracking and processing speed. At the level of
comprehensive language, fluency, reading and writing and regarding functions
executive functions show deficiencies in working memory, reasoning, planning and
decision making.

CONDUCT TO FOLLOW

Due to its stationary evolution, it is necessary to continue the treatment for


part of Occupational Therapy with a frequency of 2 times per week with the
in order to enhance cognitive functions.

WORK RECOMMENDATIONS IN THE CLASSROOM

Repeat and write instructions.


Make eye contact frequently.
Sitting Dani near the teacher's desk, this will help keep him away from
stimuli that distract him.
Create a calendar of activities and place it in a visible location.
Help Dani make his own activity calendar for later.
