DO SOLEMNLY DECLARE THAT

1. I, MATHAVARAJAH SUBRAMANIYAM, confirm that I requested SURESH PERERA


ATHUKORALA, holder of NIC No: 199329900504, to purchase a property located at
107/C/1 Lee Moala Rd, Millewa, on my behalf since I’m living in Canada.

2. The purchase price of the property was Rs. 7,000.000.00.

3. The transfer deed was registered on the 2 nd of May 2024 attested by the notary public,
Nikini Mandakini Mapitigama

4. I confirm that I, MATHAVARAJAH SUBRAMANIYAM transferred the funds to


purchase the property on several occasions to SURESH PERERA ATHUKORALA ‘S
Commercial Bank bearing Account No 1187009696- Further, I confirm that I requested
some of my friends who visited Sri Lanka to deposit funds to SURESH PERERA
ATHUKORALA ‘s account for the purchase of the property.

5. Once a person who lives in Sri Lanka, namely Jayakandan who is a friend of my
Canadian friend Suganthan also deposited Rs: 1,000.000.000 to SURESH PERERA
ATHUKORALA ‘S commercial bank account at the request of Suganthan in favor of the
said property purchase.

6. Since the purchase of the property, SURESH PERERA ATHUKORALA has taken care
of the property and maintained the property for me until I come and live in Sri Lanka
permanently.

7. I confirm that I, MATHAVARAJAH SUBRAMANIYAM transferred the funds to


purchase the property on several occasions to SURESH PERERA ATHUKORALA ‘S
Commercial Bank bearing Account #1187009696 in Sri Lanka. Further, I confirm that I
requested some of my friends who visited Sri Lanka to deposit funds to SURESH
PERERA ATHUKORALA ‘s account for the purchase of the property.

I make this Declaration for the purpose of obtaining a ‘Statement to confirm that I,
MATHAVARAJAH SUBRAMANIYAM transferred the funds to purchase the property
on several occasions to SURESH PERERA ATHUKORALA ‘S Commercial Bank
Bearing Account # 1187009696 . Further, I confirm that I requested some of
my friends who visited Sri Lanka to deposit funds to SURESH PERERA
ATHUKORALA ‘s account for the purchase of the property. and for no improper
purpose.
AND I make this solemn Declaration conscientiously believing it to be true and knowing that it
is of the same force and effect as if made under oath.
